IMPORTANT: Do not send forms to the AANS Registration Department or Housing Bureau via Federal Express, UPS, Airborne or DHL. The only overnight service that will deliver to a P.O. Box address is the U.S. Postal Service. A letter of confirmation will be sent to you via e-mail or fax (if no e-mail address is supplied) or via mail (if no e-mail or fax number is supplied) within 48 hours of receipt of your registration. You will receive a separate confirmation for your hotel reservation. All refunds will be processed and mailed following the Annual Meeting. EVALUATION FORM REWARDS PROGRAM
The Annual Meeting evaluation process is a key component in providing cutting-edge programming at the AANS Annual Meeting. AANS values medical participant feedback on the quality and diversity of its educational sessions. Your opinion helps determine future Annual Meeting programming. Evaluations will be collected on a daily basis at the convention center. To become eligible to win one of the allocated prizes, simply complete your evaluation form(s) from any of the practical clinics, breakfast seminars and scientific sessions you attend. Be sure to include your name and ID number found on your badge and drop your completed form(s) in designated Evaluation Drop Boxes located throughout the convention center or in the AANS Resource Center, booth #1120 in the exhibit hall. The more evaluations you complete, the better your chance to win! Practical clinics and breakfast seminar forms will be handed out in each session. The daily Scientific Program evaluations will be located in the Program Guide. Beginning Saturday, April 26th and concluding on Thursday, May 1st, the Evaluation Drop Boxes will be emptied daily and five lucky medical registrant prize winners will be drawn.

Prize certificates can be picked up in the AANS Resource Center, booth #1120 in the exhibit hall Monday thru Wednesday from 9:30 AM-3:30 PM. Thursday's drawing winners will be notified via e-mail. If you do not collect your certificate in Chicago, IL, it will be mailed to you after the meeting.

MEMBERS SAVE!
A convenient payment option enables members to include dues with their Annual Meeting registration. One of the privileges of AANS membership is the discounted registration rate and first option for hotels and ticketed events for this premier neurosurgical meeting. Renewal notices have been sent to all AANS members. If you have yet to pay your membership dues, you can include your dues payment with your registration payment.
