On behalf of the American Association of Neurological Surgeons (AANS) and the Congress of Neurological Surgeons (CNS), representing more than 4,000 neurosurgeons nationwide, the AANS and CNS appreciate the opportunity to comment on the Centers for Medicare & Medicaid Services’ (CMS) calendar year (CY) 2027 Medicare Physician Fee Schedule (MPFS) proposed rule, published in the Federal Register on July 16, 2026.
The proposed rule contains numerous policies with direct implications for Medicare beneficiaries’ access to complex neurosurgical care, the sustainability of neurosurgical practices, and physicians’ ability to participate meaningfully in quality programs. The AANS and CNS support CMS’s objectives of improving payment accuracy, aligning Medicare payment with the resources required to furnish services, and promoting meaningful, clinically relevant quality measurement.
